Job Overview We are seeking a dedicated Registered Nurse (RN) to join our team remotely in a part-time capacity specializing in Chronic Care Management under a 1099 independent contractor arrangement. This role offers flexibility and the opportunity to provide high-quality patient care across diverse settings, including long-term care, assisted living, and home health environments. The ideal candidate will possess extensive clinical experience, proficiency with electronic health record (EHR) systems such as Epic, Cerner, and Athenahealth, and a strong background in managing complex chronic conditions. This position emphasizes patient-centered care, health coaching, and adherence to HIPAA standards, with an emphasis on improving health outcomes through comprehensive assessment and management.
Duties
Conduct thorough patient assessments via telehealth platforms
Manage chronic conditions such as cardiac issues, diabetes, respiratory illnesses, and neurological disorders through personalized care plans.
Utilize EMR/EHR systems like Epic, Cerner, and eClinicalWorks for documentation of patient interactions, care plans, and progress notes.
Coordinate care with multidisciplinary teams including physicians, specialists (e.g., ophthalmology, neurology), and community resources to ensure continuity of care.
Provide health coaching focused on disease management, medication adherence, lifestyle modifications, and preventive care strategies.
Monitor patients' responses to treatments.
Educate patients and families about managing complex health conditions including dementia/dementia care, Alzheimer's care, hospice & palliative medicine needs.
Ensure compliance with HIPAA regulations during all patient interactions and documentation processes.
Stay current with healthcare standards such as NCQA standards and ICD coding updates (ICD-9/ICD-10).
Document all patient encounters accurately in medical records while maintaining confidentiality and security protocols.
Experience
Written and spoken bilingual preferred
Proven experience in various clinical settings including chronic care management and remote patient management
Extensive background in managing chronic diseases such as heart failure, COPD/asthma (pulmonary), renal dialysis patients, or post-acute care populations.
Experience working with diverse populations including geriatrics (memory care/dementia), pediatrics (neonatal/infant/toddler), behavioral health patients or individuals with disabilities.
Knowledge of medical terminology and physiology to accurately assess patient needs and interpret diagnostic evaluations like sonography or laboratory results.
Skilled in using EMR/EHR systems such as EPIC, Athenahealth, and eClinicalWorks
Prior experience in case management programs
